Case Coordinator, Outreach, Senior Care
1 Year Contract (Maternity Leave)
Flexible work hours may include some evenings and weekends
North York Seniors Centre provides programs and services to promote the physical, emotional and social well-being of adults 55+ living in our diverse community.
This position reports to the Supervisor, Client Services.
Participate in continuous quality improvement processes and assist Outreach Team with research and development of best practices
Promote and maintain a high standard of customer service that is client focused
Provide all aspects of case coordination to NYSC clients including case management, linkage, referral, information and supportive counselling. Including intake responsibilities.
Provide caregiver support in an individual and group setting including participation in and facilitation of caregiver training sessions
Participate in internal and external groups and networks to maintain and enhance NYSC position in the greater community as required
Maintain statistical records and ensure client files are updated
Education: Social Service Worker or Bachelor of Social Work
Background in Mental Health, an asset
Experience: Minimum 2 years’ experience in a community support setting
Other: Access to a reliable vehicle required
Strong administrative and computer (Windows) skills including GoldCare and interRAI CHA
Resourceful and a creative problem solver
Excellent group facilitation skills
Strong interpersonal and customer service skills
Familiarity working with and supporting volunteers
Ability to communicate effectively in English (verbal and written); other languages, an asset
Ability to be a self-starter, able to work independently as well as in a team
Physically able to assist with lifting and moving equipment, tables and chairs
